Description: A US jumper at a boggie run by a US DZ, on the first load of the boogie, deployed her reserve too low for it to open in time. After a 12way, with a 4500 breakoff altitude, it is speculated that she could not find her BOC throwout, pulled her cutaway handle, then her reserve. It is likely she had recently transitioned from a ROL throwout.
Lessons:You should practice any transition of deployment or emergency procedures a zillion times. You may look odd, but do it whenever you think of skydiving. Just reach back to where you think the handle should be, and pretend to throw it. Do that 10 times a day for a few weeks, and it'll be totally ingrained. An AAD would have almost certainly helped this jumper.
